The Clinical Observation of 113 Patients with Thrombosis Obliterans Treated by GlucoNature Since 1967, the application of GlucoNature treatment of thrombosis obliterans has been carried on in our hospital. In 1976, we made a report on the clinical curative effect of Gluconature. We found that the clinical curative effect of Gluconature formula wine group is much better than other similar products. Then, we have been taking GlucoNature as the main treatment under Syndrome differentiation and treatment, and we have cured 113 cases of patients with thromboangiitis obliterans from 1976 April to 1978 March. Clinical treatment as follows:
Clinical Data
Gender: male (113 cases )
Age: 20-30 years old in 38 cases, 31- 40 years old in 39 cases, 41-50 years old in 31 cases, over 51 years old in 5 cases.
Occupation: peasants in 57 cases, workers in 31 cases, soldiers in 6 cases, teachers in 6 cases.
Duration of illness: 1-6 months in 23 cases, 7 months -1year in 14 cases, 1-3 years in 24 cases, 3-5 years in 18 cases, 6-10 years in 18 cases, over than 10 years in 16 cases. Disease cause: long-term smoking in 47 cases, smoking and the cold aspic in 36 cases, smoking and the injured in 17 cases, 4 cases were wounded, cold outside the frozen in 2 cases, 7 cases of unknown cause. The clinical stages: the first stage (local ischemic stage) in 34 cases，second stage (dystrophie stage) in 30 cases，third stage （necrosis stage ）in 49 cases（grade 1 putrescence in 25 cases，grade 2 putrescence in 21 cases，grade 3 putrescence in 3 cases） Therapeutic method: GlucoNature wine: dry unprocessed materials of Gluconature, grind them for fine, cut them into pieces and make them into crude powder, soak them with white spirit (55%) for 15 days. Gluconature wine (5-10%) will be made. Take 20-30 milliliter per day, three times a day. According to the condition, take prescription below according to the condition
A. For the ones who suffer from dampness-heat pouring down, the method of clearing heat and draining dampness is advised. Four Wonderful Herbs Resting Hero Decoction is better to be taken orally: Honeysuckle Flower and Yuan shen (30 grams of each), Chinese Angelica, Red Peony, Medicinal Cyathula Root (15 grams of each.), Phellodandron Bark, Amur Cork-Tree, Cape Jasmine, Weeping Forsythia Capsule, Atractylodes Rhizome, Lithospermum Root and Liquorice Root (10 grams of each.), Safflower and Armand Clematis Stem (6 grams of each.). The ones who suffer from heavy infection of thanatosis are advised to take 2 doses per day. The following prescription can be taken for the ones who suffer from concurrency phlebitis orally: 60 grams of Honeysuckle stem, 30 grams of Salvia Root, Phellodandron Bark, Medicinal Cyathula Root, Atractylodes Rhizome (10 grams of each). For the ones who have cold feet, 1.5 grams of Cassia Bark can be added.
B. For the ones who have both Qi and Blood deficiency, the method of tonifying Qi and nourishing Blood is advised, take the following prescription orally: a, 60 grams of Milkvetch, Salvia Root and Honeysuckle Flower (30 grams of each) b, Compound formula white flower Salvia Root pills: composed of Milkvetch and white flower Salvia Root (250 grams of each). Grind them for fine, cut them into pieces and make them into thin powder, and then make them into pills with water and the size is almost the same as the mung bean, make them dry. Take 10-15 grams per time, 3 times per day. For general patients, arteries pills are popular. 180 grams of Salvia miltiorrhiza, 90 grams of each Red Peony and Poria, Chinese Angelica, Honeysuckle Flower, Danpi and Isatis Root (60 grams of each). Ligusticun, Peach Seed, Medicinal Cyathula Root and Wax gourd (60 grams of each), grind them for fine, cut them into pieces and make them into thin powder, and then make them into pills with water and the size is the same as the mung bean, make them dry. Take 10-15 grams, three times a day.
External treatment: used for thrombotic shallow phlebitis
A. Grind Rice bean for fine, make them into thin powder and then dress them on the wound with eggs.
B. Take the same amount of Amur Cork-Tree, Phellodandron Bark , and Rhubarb, grind them for fine, dress them on the wound with water and honey. For the ones who suffer from toe thanatosis, carry on the Resection of the necrotic tissue when the bad part is clear, change fresh dressing for the wound routinely.
Results:
There are totally 113 cases of patients with thromboangiitis obliterans patients in our clinical observation, according to the clinical standard of Curative effect due to “the blood living obliterans control handbook”, clinical cure in 28 cases (24.8%), significant improvement in 61 cases (53.9%), progress in 20 cases (17.17%), the total effective rate was (96.4%), ineffective in 4 cases (3.6%), including 1 case of patient whose gangrene extended to execute knees crus ankle, and the patient received below knee amputation in the end , the amputation rate was 0.9%, 3 cases were treated by more than two months with symptoms and signs of no improvement .
Discussion
The history of the application with GlucoNature to treat thrombosis obliterans extends over 80 years. We find that there are certain curative effects on Patients by taking Gluconature: the function of analgesia is significant, improving blood circulation well, good looks on patients, better sweatiness, toenail growing, which indicates that GlucoNature has the function of expanding the peripheral vascular. But for the patients with heavy gangrene thromboangiitis obliterans, taking GlucoNature wine only will not take effect well, they should take TCM herbs as well for orally according to the internal condition and Syndrome differentiation and treatment. For Cold Yin vasculitis, the method of warming and blood-activating is advised, taking Prepared Common Monkshood Daughter Root, Dry Ginger, Cassia Twig, and Compound formula Salvia Root pills is better to be added. For Blood stasis syndrome vasculitis, the method of Blood –activating and stasis –resolving is needed, the curative effect will be better with increasing dosage of Gluconature wine and taking the pulse-invigorating pill as well. For dampness-heat poring down vasculitis, the method of clearing heat and draining dampness have to be taken. Four Wonderful Herbs Resting Hero Decoction will be better if taken orally, after getting better, the patients can take the pulse-invigorating pill to replace Four Wonderful Herbs Resting Hero Decoction. For heat toxin vasculitis, patients with heavy gangrene thromboangiitis obliterans can take 2 dosages of Four Wonderful Herbs Resting Hero Decoction, or small dosage but many times. For both QI and Blood deficiency vasculitis, the method of strengthening Qi and nourishing blood should be taken. Decocting Salvia Root and Astragalus is better to advance the wound healing. Compound formula Salvia Root can be taken during recovery phase. The healing effect of taking Gluconature wine with TCM medicine is much better than taking TCM medicine alone, maybe there is something to do with the function of expanding the peripheral vascular of GlucoNature wine itself. There are totally 113 cases of patients with thromboangiitis obliterans patients in this group, the healing rate and improvement rate is 78.7% after clinical treatment, the amputation rate is 0.9%. The curative effect is satisfied. For thrombophlebitis leg, take medicine of clearing heat and draining dampness, blood-activating and stasis resolving orally combined with external treatment can make pains erythematous fadeaway, therefore, external treatment should be paid enough attention. Resection of the necrotic tissue ought to be carried on when pantomime toe becomes black and thanatosis and the bad part is clear enough, the wound can heal successfully after treatment, in addition, and most patients are able to save their legs and avoid amputation.
